if you are already a client of ours and have just added a new pet to your family, you only need to fill this one form out

Once you’ve completed your forms, you have a few options:
       1. Bring the completed forms with you to your next appointment
       2. Email the completed forms to us at requests@encinavet.com with the subject “Client Registration Form”
       3. Fax the forms in to us (925) 937-8514
