We ask that you please take the time to fill this form out entirely before your first appointment with behaviorist, Dr. Meredith Stepita, to ensure your time is well spent while in the appointment.

Once the form is filled out, please save it to your computer. Begin composing an email and add your completed form as an attachment. Send the email to drstepita@encinavet.com with the subject being “Completed Behavior Form for Patient Name,” replacing Patient Name with your pet’s first and last name.
